The following information will be helpful in making an appointment to be seen for a urologic problem. If you are making the appointment yourself . . . * Telephone us at (205) 877-2860 Should you be referred by another physician, he or she may set up the appointment for you and advise you accordingly. Regardless of the circumstances of who makes your appointment, the following applies. These are items that you should bring along with you on the day of your visit. * Your insurance card * Physician referral forms if required by your insurance company * A list of all current prescription medications and/or all over-the-counter medicines that you take. (We need to know the strength, how many, and how often you take them.) * Pertinent information concerning your medical history, including any surgeries you have undergone in the past. * Any recent X-rays, any other copies of medical reports including lab work. ATTENTION: NEW PATIENTS -- If this is your first visit, please see the Patient History Form and Patient Registration Form that you will need to complete in advance of your visit and bring along with you to help ensure that you will be seen in a timely manner.
